Drug Use and Overdose in Cuyahoga County

The picture in Greater Cleveland has been improving. Overdose deaths have fallen nationally for three years running, and Cuyahoga County has seen one of the sharper local declines, driven by a drop in fentanyl-involved deaths. The county’s Medical Examiner has cautioned that the crisis is still evolving rather than over: as of the office’s 2026 update, cocaine has passed fentanyl as the drug most often involved in local overdose deaths, and fentanyl continues to turn up mixed into other drugs, which is why naloxone and drug-checking still matter.

Naloxone and Harm Reduction

Naloxone reverses an opioid overdose and is safe to give even if you are not certain opioids are involved. In Ohio, anyone can legally carry and administer it, and it is widely available for free. Local and statewide options include:

• Project DAWN (Ohio Department of Health): free naloxone kits and overdose response training at more than 500 sites statewide, no prescription needed.
• MetroHealth Project DAWN, Cleveland: a mobile unit and a harm reduction vending machine with free naloxone outside the main campus emergency department.
• ADAMHS Board of Cuyahoga County: free naloxone and fentanyl test strips at vending machines around the county.
• By mail: Harm Reduction Ohio and NEXT Distro mail naloxone to Ohio residents at no cost through nextdistro.org/ohio.
• Pharmacies: naloxone nasal spray is sold over the counter, without a prescription.

Ohio’s Good Samaritan law offers limited protection from minor drug possession charges for someone who seeks emergency help during an overdose.
